New technical and scientific developments have been advocated to promote the success of the endodontic treatment. In addition to rotary and reciprocating systems, irrigating solution agitation has been suggested and passive ultrasonic irrigation (PUI) is the most used. OBJECTIVE: To evaluate, in vitro, the effect of ultrasound streaming (US) in the disinfection of flattened root canal systems prepared by the ProTaper, BioRaCe and Reciproc systems, utilizing the microbiological culture. METHODOLOGY: Extracted human mandibular incisors (n=84) were used. Suspensions of Enterococcus faecalis (ATCC 29212) were standardized and inserted along with the teeth immersed in brain-heart infusion (BHI) broth. The contamination was made following a protocol during 5 days. The teeth were randomly divided into six groups: G1, ProTaper Universal; G2, ProTaper Universal with US; G3, BioRaCe; G4, BioRaCe with US; G5, Reciproc; and G6, Reciproc with US. Irrigation was performed with saline solution. After biomechanical preparation, microbiological samples were performed with sterilized paper points, which were diluted and spread on BHI agar; after 48 h, the colony forming units (CFU/mL) were counted for each sample. RESULTS: Groups using ultrasonic agitation presented a greater antibacterial effect than the other ones, even using saline solution as irrigant. The ProTaper Universal system showed the best antibacterial activity of the tested systems (median of 0 CFU/mL with and without surfactant or ultrasonic activation [PUI]). Even with PUI, Reciproc (median of 2.5 CFU/mL with PUI and 5 without it) could not reduce as many colonies as ProTaper Universal without US. The BioRaCe system had greater bacterial reduction when using US (median of 0 CFU/mL with PUI and 30 without it). CONCLUSIONS: US promoted greater reduction in the number of bacteria in the flattened root canals prepared with nickel-titanium mechanized systems. Regarding the instruments used, the ProTaper Universal system was the most effective in reducing the bacterial number.

The aim of this case report is to describe the treatment of a 9-year-old patient who suffered external root resorption of the permanent maxillary left lateral incisor following reimplantation of the avulsed left central and lateral incisors. Sixteen days after reimplantation and splinting of the incisors in a hospital emergency department, the patient was brought to the pediatric department of a dental school for further treatment. Root canal access was created in the maxillary left lateral and central incisors, and calcium hydroxide paste was used as intracanal dressing. At the 5-month follow-up, a radiograph revealed extensive external root resorption, a communicating root canal, and a periodontal lesion affecting the left lateral incisor. Management of the root resorption included obturation of the apical third of the canal with gutta percha and the middle third with mineral trioxide aggregate (MTA). At the 3-year recall examination, the patient was asymptomatic, and no mobility or soft tissue alterations were observed clinically. There was no radiographic sign that resorption had progressed. Despite the success of treatment, observation is still required. The use of MTA may be considered an alternative treatment for external root resorption after tooth reimplantation. The technique may allow tooth preservation in children until skeletal growth and development are completed and implant treatment may be considered.

The aim of this study was to evaluate the subcutaneous tissue response in rats and the antimicrobial activity of intracanal calcium hydroxide dressings mixed with different substances against E. faecalis. Fifty four rats were divided into three experimental groups according to the vehicle in the calcium hydroxide treatment: 0.4% chlorohexidine in propylene glycol (PG),Casearia sylvestris Sw in PG and calcium hydroxide+PG (control group). The pastes were placed into polyethylene tubes and implanted into the subcutaneous tissue. After 7, 14 and 30 days, the samples were processed and histologically evaluated (hematoxylin and eosin). The tissue surface in contact with the material was analyzed, and the quantitative analysis determined the volume density occupied by the inflammatory infiltrate (giant cells, polymorphonuclear cells and mononuclear cells), fibroblasts, collagen fibers and blood vessels. For the antimicrobial analysis, 20 dentin blocks infected with E. faecalis were treated with calcium hydroxide pastes in different vehicles; 0.4% chlorhexidine in PG, PG, extract fromCasearia sylvestris Sw in PG and a positive control (infection and without medication) for 7 days. The efficiency of the pastes was evaluated by the live/dead technique and confocal microscopy. The results showed that 0.4% chlorhexidine induced a higher inflammatory response than the other groups. The Casearia sylvestris Sw extract showed satisfactory results in relation to the intensity of the inflammatory response. In the microbiological test, there were no statistical differences between the evaluated intracanal dressings and the percentage of bacterial viability was between 33 and 42%. The control group showed an 86% viability. Antimicrobial components such as chlorhexidine or Casearia sylvestris Sw did not improve the antimicrobial activity against E. faecalis in comparison to the calcium hydroxide+PG treatment. In addition, the incorporation of chlorhexidine in the calcium hydroxide paste promoted the highest inflammatory response.

Objective: To evaluate the response of rat subcutaneous tissue in implanted polyethylene tubes that were filled with GMTA Angelus and Portland cements containing different arsenic concentrations. Material and Methods: Atomic absorption spectrophotometry was utilized to obtain the values of the arsenic concentration in the materials. Thirty-six rats were divided into 3 groups of 12 animals for each experimental period. Each animal received two implants of polyethylene tubes filled with different test cements and the lateral of the tubes was used as a control group. After 15, 30 and 60 days of implantation, the animals were killed and the specimens were prepared for descriptive and morphometric analysis considering: inflammatory cells, collagen fibers, fibroblasts, blood vessels and other components. The results were analyzed utilizing the Kuskal-Wallis test and the Dunn's Multiple test for comparison (p<0.05). Results: The materials showed, according to atomic absorption spectrophotometry, the following doses of arsenic: GMTA Angelus: 5.01 mg/kg, WPC Irajazinho: 0.69 mg/kg, GPC Minetti: 18.46 mg/kg and GPC Votoran: 10.76 mg/kg. In a 60-day periods, all specimens displayed a neoformation of connective tissue with a structure of fibrocellular aspect (capsule). Control groups and MTA Angelus produced the lower amount of inflammatory reaction and GPC Minetti, the highest reaction. Conclusions: There was no direct relationship between the concentration of arsenic present in the composition of the materials and the intensity of the inflammatory reactions. Higher values, as 18.46 mg/kg of arsenic in the cement, produce characteristics of severe inflammation reaction at the 60-day period. The best results were found in MTA angelus. .

O objetivo deste estudo foi avaliar o pH e a liberação de íons cálcio dos cimentos AH Plus, MTA Fillapex e Sealapex, em diferentes períodos, quando empregados em obturações de canais radiculares, bem como, a qualidade das obturações, após 1 ano de realizadas. O pH e a liberação de íons cálcio foram avaliados por meio de um peagômetro e espectrofotômetro de absorção atômica, respectivamente. A qualidade das obturações foi avaliada por meio de um estereomicroscópio. Cento e vinte e quatro pré-molares inferiores humanos extraídos, unirradiculados, foram divididos em 01 grupo controle e 02 grupos experimentais, de acordo com a padronização do diâmetro do forame apical. Em seguida, os 02 grupos experimentais foram divididos em 4 subgrupos, considerando as técnicas de instrumentação e obturação empregadas. Após o preparo dos canais radiculares cada subgrupo teve o número de dentes divididos por dois, formando novos subgrupos, de acordo com os cimentos obturadores utilizados. Imediatamente após a obturação, a raiz de cada dente foi imersa em 15 mL de água ultrapura para avaliação do pH e liberação de íons cálcio nos seguintes períodos: 1 h, 1, 3, 7, 30, 180 e 365 dias, após a obturação. Após o período de 1 ano, os dentes foram seccionados transversalmente a 2, 4, 6 mm do forame apical para análise da qualidade das obturações, analisando-se a porcentagem de cimento, guta-percha e espaços vazios na massa obturadora. Os cimentos apresentaram pH neutro, porém, aos 180 dias, os valores mostraram um ligeiro aumento quando utilizados em canais com forames de diâmetro 0,25 mm. Quando utilizados em canais com forames de diâmetro 0,45 mm, o pH mostrou-se com valores maiores para todos os períodos sem distinção do cimento. Todos os cimentos liberaram íons cálcio, com menores valores para o AH Plus. Na estereomicroscopia, observaram-se variações nas porcentagens de cimento e guta-percha, considerando-se os diâmetros foraminais e técnicas obturadoras, e porcentagem...

After the preparation of the root canals, each subgroup had the number of teeth divided by two, forming new subgroups, according to the sealers used. Immediately after the obturation, the root of each tooth was immersed in 15 mL of ultrapure water for the evaluation of the pH and calcium ion release in the following time periods: 1 hr, 1, 3, 7, 30, 180 and 365 days after the obturation. After the period of one year, the teeth were transversely sectioned at 2, 4 and 6 mm from the root apex to examine the quality of the obturations, analyzing the percentage of cement, gutta-percha and voids in the obturator mass. The cements presented a neutral pH, which showed slightly increase as of 180 days when used in canals with foramens of 0.25 mm diameters. When used in canals with foramens with diameters of 0.45 mm, the pH presented higher values in all periods for all cements. All cements released calcium ions, with lower values for the AH Plus. In the stereomicroscopy, there were variations in the percentages of cement and gutta-percha, considering the foraminal diameters and obturation techniques and an insignificant percentage of voids.

OBJECTIVES: To investigate if there is a relation between the increase of bismuth oxide and the decrease of pH levels and an intensification of toxicity in the Portland cement. MATERIAL AND METHODS: White Portland cement (WPC) was mixed with 0, 15, 20, 30 and 50% bismuth oxide, in weight. For the pH level test, polyethylene tubes were filled with the cements and immersed in Milli-Q water for 15, 30 and 60 days. After each period, the increase of the pH level was assessed. For the biocompatibility, two polyethylene tubes filled with the cements were implanted in ninety albino rats (n=6). The analysis of the intensity of the inflammatory infiltrate was performed after 15, 30 and 60 days. The statistical analysis was performed using the Kruskal-Wallis, Dunn and Friedman tests for the pH level and the Kruskal-Wallis and Dunn tests for the biological analysis (p<0.05). RESULTS: The results showed an increase of the pH level after 15 days, followed by a slight increase after 30 days and a decrease after 60 days. There were no significant statistical differences among the groups (p>0.05). For the inflammatory infiltrates, no significant statistical differences were found among the groups in each period (p>0.05). The 15% WPC showed a significant decrease of the inflammatory infiltrate from 15 to 30 and 60 days (p<0.05). CONCLUSIONS: The addition of bismuth oxide into Portland cement did not affect the pH level and the biological response. The concentration of 15% of bismuth oxide resulted in significant reduction in inflammatory response in comparison with the other concentrations evaluated.

INTRODUCTION: The purpose of this study was to evaluate the antimicrobial activity of calcium hydroxide, 2% chlorhexidine gel, and triantibiotic paste (ie, metronidazole, minocycline, and ciprofloxacin) by using an intraorally infected dentin biofilm model. METHODS: Forty bovine dentin specimens were infected intraorally using a removable orthodontic device in order to induce the biofilm colonization of the dentin. Then, the samples were treated with the medications for 7 days. Saline solution was used as the control. Two evaluations were performed: immediately after the elimination of the medication and after incubation in brain-heart infusion medium for 24 hours. The Live/Dead technique (Invitrogen, Eugene, OR) and a confocal microscope were used to obtain the percentage of live cells. Nonparametric statistical tests were performed to show differences in the percentage of live cells among the groups (P < .05). RESULTS: Calcium hydroxide and 2% chlorhexidine gel did not show statistical differences in the immediate evaluation. However, after application of the brain-heart infusion medium for 24 hours, 2% gel chlorhexidine showed a statistically lesser percentage of live cells in comparison with calcium hydroxide. The triantibiotic paste significantly showed a lower percentage of live cells in comparison with the 2% chlorhexidine gel and calcium hydroxide groups in the immediate and secondary (after 24 hours) evaluations. CONCLUSIONS: The triantibiotic paste was most effective at killing the bacteria in the biofilms on the intraorally infected dentin model in comparison with 2% chlorhexidine gel and calcium hydroxide.

INTRODUCTION: The aim of this study was to evaluate the pH, calcium ion release, setting time, and solubility of white mineral trioxide aggregate (WMTA) and white Portland cement (WPC) combined with the following radiopacifying agents: bismuth oxide (BO), calcium tungstate (CT), and zirconium oxide (ZO). METHODS: Fifty acrylic teeth with root-end filling material were immersed in ultrapure water for measurement of pH and calcium release (atomic absorption spectrophotometry) at 3, 24, 72, and 168 hours. For evaluation of setting time, each material was analyzed according to the American Society for Testing and Materials guidelines 266/08. The solubility test was performed according to American National Standards Institute/American Dental Association specification no. 57/2000. Solubility, setting time, and pH values were compared by using analysis of variance and Tukey test, and the values of calcium release were compared by the Kruskal-Wallis and Miller tests. The significance level was set at 5%. RESULTS: The pH and calcium release were higher at 3 and 24 hours. WPC was the material with the higher values for both properties. WMTA had the greatest solubility among all materials (P < .05). All radiopacifiers increased the setting time of WPC, and WMTA had the shortest setting time among all materials (P < .05). CONCLUSIONS: All materials released calcium ions. Except for WPC/CT at 168 hours, all materials promoted an alkaline pH. On the basis of the obtained results, ZO and CT can be considered as potential radiopacifying agents to be used in combination with Portland cement.

O MTA (Agregado Trióxido Mineral) tem sido utilizado na endodontia com diferentes finalidades terapêuticas. Essencialmente ele é constituído pelo cimento Portland acrescido de um radiopacificador. Entre seus componentes, é destacada a presença do arsênico, o qual sofre variação de concentração em função da origem do calcário. A dúvida é que se diferentes concentrações de arsênico representam alguma influência na sua biocompatibilidade. O objetivo deste estudo foi avaliar, microscopicamente, a reação tecidual em subcutâneo de ratos frente a cimentos Portland contendo diferentes concentrações de arsênico. Foram utilizados 36 ratos Wistar albinos, distribuídos em 4 grupos experimentais segundo o material empregado perfazendo um total de 6 implantes por período (15, 30 e 60 dias). A lateral dos tubos serviram como grupo controle. Após 15, 30 e 60 dias, os animais foram mortos e os espécimes foram preparados histotecnicamente para análise microscópica. Os dados da avaliação morfométrica foram submetidos à análise de variância a dois critérios (ANOVA) e teste de Tukey para a comparação (p<0,05). Os resultados mostraram uma maior densidade de volume de células inflamatórias aos 15 dias, com redução dessa densidade com o passar do tempo para todos os materiais. Os resultados da análise descritiva mostraram inflamação crônica induzida pelos materiais, com intensidade de leve a moderada, e organização de uma cápsula fibrosa ao redor de todos os espécimes e em todos os períodos. Os cimentos analisados produziram respostas inflamatórias semelhantes, mesmo apresentando quantidades diferentes de arsênico na composição, apenas com diferença estatisticamente significante entre o cimento Portland cinza Votoran e os demais cimentos nos três períodos estudados.Título em inglêsMicroscopic evaluation of the tissue reaction in subcutaneous of rats to Portland cements containing different.


The MTA (Mineral Trioxide Aggregate) has been used in Endodontics for different therapeutic purposes. Essentially, it is composed of Portland cement with addition of a radiopacifier. Its components include arsenic, whose concentration is variable according to the origin of the calcareous. There is doubt whether different arsenic concentrations might influence its biocompatibility. This microscopic study evaluated the reaction of subcutaneous tissue of rats to Portland cements containing different arsenic concentrations. The study was conducted on 36 albinus Wistar rats, divided in 12 animals for each study group. Each animal received two implants of polyethylene tubes, completely filled with the test cements, and the lateral aspect of the tubes was taken as control group. The animals were killed after 15, 30 and 60 days and the specimens were submitted to histotechnical preparation for microscopic analysis. Data of the morphometric analysis were submitted to two-way analysis of variance (ANOVA) and the Tukey test for comparison (p<0.05). The results evidenced greater density of volume of inflammatory cells at 15 days, which was reduced with time for all materials. The results of this descriptive analysis demonstrated chronic inflammation induced by the materials, of mild to moderate intensity, and organization of a fibrous capsule around all specimens and in all periods. The cements induced similar tissue responses, despite the different arsenic concentrations in their composition.
